• a 6month old Baby, mixed.
• to be a photo double for Chicago Med on Friday, April 8th.
• Put your child's height in inches, weight, and when they were born.

There will be a covid test Wednesday, April 6th.

• Parents MUST be fully vaccinated and boosted
• Let us know if you need a Joan Philo work permit

Important note: 
For the health and safety of all individuals on "Chicago MED," all “Zone A” cast and crew must be fully vaccinated against COVID-19 and a booster prior to beginning work.

• You must have had two shots two weeks prior to today's date.
• Parents must all be fully vaccinated. Parents must be boosted too.
